CACCC 10th Anniversary Celebration Events – 2015

CACCC held its 10th Anniversary Celebration Fundraising Event at Milpitas on 12/5/2015. Invited 2 keynote speakers from Taiwan: Dr. Co-Shi Chantal Chao, RN, PhD. and Grace Pao Fang Chang.  Other 10th Anniversary Events including: two Health Professional Forums, two Public Forums, one 30-Hour End of Life Care Training and Two One-Day Volunteer Trainings.

 

Held 30-Hour Hospice & Palliative – 2015, 2012 & 2011

Held one 30-Hour Hospice & Palliative Care Training at Methodist Hospital in Southern California in 2015, at San Francisco in 2012, at El Camino Hospital in 2011 and at City of Hope in Southern California in 2011.

Published CACCC’s first book “Learning to Let Go: Saying Goodbye Peacefully”.

 

Heart to Heart ® Café - 2014

CACCC's Heart to Heart® cards are designed to facilitate communication about issues surrounding the end of life. There is a two-fold emphasis in the Heart to Heart communication activity: decide what you want at the end of life and complete the Advance Health Care Directive, so everybody knows what your wishes are. Held 48 Heart to Heart® Cafés in 2014, 45 Cafés in 2015 and 53 Cafés in 2016.

Understanding Hospice Video- 2011

As part of a contract with the Hospice Foundation of America, the CACCC has created a webcast for Chinese Americans about hospice care in the U.S. The film features multiple perspectives, from families and elders in the Chinese community to doctors, chaplains, nurses, social workers, therapists, and volunteers. The Webcast is available free of charge from the HFA website (http://www.hospicefoundation.org/) and from our website.
