Video project with Nous Foundation – 2016
Worked with the Nous Foundation to help them make culturally appropriate videos for the Mandarin speaking population on Advance Care Planning. These videos will be available on the ACP Decisions website.
Interpreted and translated 14 scripts, facilitated the Nous Foundation’s ability to obtain footage that could potentially be used as B-roll footage for Foundation videos of Chinese patients and/or family members, and provided the Foundation with an experienced health professional narrator for the Mandarin video content.
Palo Alto Medical Foundation Community Health Care Endowment Fund – 2016 Project
CACCC provides education outreach about Advance Heath Care Directive (AHCD), Physician Orders for Life-Sustaining Treatment (POLST) and End of Life issues to Chinese Americans residing in Alameda, Oakland, San Mateo and Santa Clara counties. The CACCC conducts community outreach, particularly to Chinese seniors, on the advantages of completing an AHCD and the POLST form. CACCC seminars explain what these forms do and how to complete them. CACCC volunteers help participants actually complete their AHCD at the seminars using the Chinese translation of the forms as a tool for understanding the implications of each section of the AHCD and the POLST form. CACCC outreach also includes Heart to Heart Cafés, where participants will gain a better understanding of end of life issues, such as the benefits of hospice and palliative care.
